Multi-Factor Authentication (MFA), Forgot or Lost Phone or FOB
Important Information
If you:
- Lost or broke your phone,
- Lost or broke your MFA token/FOB,
-
Forgot your MFA token/FOB
- Forgot your phone or do not have it with you
And cannot access your account because you are unable to authenticate the MFA notification request
contact the Service Desk 970-221-6791
Further Knowledge
If you have an employee that has left the City and is no longer an employee, and they had an MFA Token/FOB please submit a request ticket to Forfeit the FOB so it can be returned to IT.
